Co-payments

Our patients are expected to present their insurance card (if applicable) at each visit. All co-payments and past balances are due and payable at the time of service. Our office accepts cash, checks, Visa and MasterCard.

Returned Checks

Checks returned from the bank for insufficient funds will be charged a $25 reprocessing fee.

Self Pay Accounts

Self pay accounts are patients who are covered by insurance plans in which our office does not participate, patients without an insurance card on file, or who at the time of service have not met their deductible. For these accounts, payment is expected in full at the time of service, including surgeries.

Non-Participating Insurance Plans

The financial obligations of patients who are insured by companies with which this practice does not participate are considered self-pay accounts. As a courtesy, we will file your insurance as a non-assigned claim with any insurance payments reimbursing you directly. You are expected, however, to pay for the services in full at the time of your visit.

  • Must be accompanied by Custodial Parent
  • Legal guardians must present custody papers
  • Divorced Parent Policy - medical bills are to be paid by the parent who brings the child for treatment, regardless of the conditions of a divorce decree.
